A Systems Change Project

The Alzheimer's Association of Northeastern New York and the Fulton County Office for Aging are partnering on a federal grant aimed at improving access to care for individuals and families living with Alzheimer's disease. The alliance is one of only 20 county- agency proposals in the United States to receive the three- year, $170,000 grant, awarded by the Department of Health and Human Services' Administration on Aging.

Fulton County has been at the forefront of a Point of Entry System with its already established Central Assessment Program. Consistent with a "one stop shop", Single Point of Entry provides access to community agencies which provide services to maintain individuals in their homes.

The grant's mission is to assist individuals with Alzheimer's disease to function both in the home and in the community. The Alzheimer's Association will be providing free training on caregiving for families dealing with the disease. Training for professional healthcare workers in Fulton County will also be provided in order to develop a dementia capable workforce. Services such as homecare and day programs will also be available for individuals with dementia.

Alzheimer's staff will be available weekly at Fulton County Central Assessment and Placement located at 4 Daisy Lane in Johnstown, NY.

Alzheimer's Disease

Alzheimer's disease and other related dementias are neurological disorders that affect over four million Americans nationwide. Those who are afflicted experience impaired memory, thinking, and communication. In many cases, these conditions leave individuals totally unable to care for themselves and eventually leads to death. Alzheimer's disease is the 4th leading cause of death in adults and currently ranks as the third most expensive illness in the United States. It is important to acknowledge that this disease also affects the families of persons afflicted. The emotional, social, and financial costs of caregiving can be overwhelming. We are here to help.

Family Support

The task of caregiving for someone with Alzheimer's disease may become stressful and overwhelming. Monthly support groups for families are held throughout our service area. Here families will find people who know about the fear, exhaustion, and even the anger that you sometimes feel when you have a loved one afflicted with the disease. Central Assessment and Placement of Fulton County

The Fulton County Central Assessment Program (CAP), working in collaboration with the Fulton County Office for Aging and the Department of Social Services, is a multi-agency program providing an array of services to the community of Fulton County. CAP provides a central entry point to long term care services for both in- home care and nursing home care. The primary goal of CAP is to maintain a client in the home whenever possible with a developed care plan utilizing family, friends, and community supports. Anyone, regardless of age or income, is entitled to receive a social, health, and financial assessment at no charge. and Placement of Fulton County

Who Can CAP Help?

Any resident of Fulton County is eligible for Central Assessment's evaluation to determine long term care needs. This service is available for both Medicaid and Non- Medicaid clients. Some of the services CAP can assist with include adult day and in- home services, information on Medicaid eligibility, information on health services, and referrals to other sources of community support.

The Fulton County Office for Aging in cooperation with the NYS Office for Aging, the Center for Excellence in Aging at SUNY Albany, and the Alzheimer's Association of Northeastern New York are pleased to offer a new opportunity for families and caregivers of people with dementia through the Coordinated Care Alzheimer's Demonstration Project.
